To enroll at César Chávez Academy, you will need to download the PRE-REGISTRATION on this page, fill it out, and bring it to the main office of your school (Vernor, Martin, Middle School, or High School).
 
Along with the PRE-REGISTRATION, you will need to bring the following required forms:

1. Birth certificate

2. Immunization Record 

3. Current and Last Report Card

4. Parent(s)/Guardian(s) Identification Card

5. Health Appraisal if playing sports

Para inscribirse en La Academia César Chávez, tendrá que descargar la Pre-Registración en esta página , llénelo y tráigala a la oficina principal de su escuela (Vernor, Martin, Middle School, o High School).
